Telehealth Informed Consent
By using our services, you consent to receive medical care via telehealth from a licensed physician affiliated with Longevity Health Medical Group PA, a physician-owned professional medical corporation. All clinical consultations and prescriptions are handled exclusively by US board-certified medical practitioners licensed to practice in Florida.
1. Nature of Telehealth Services
Telehealth involves the use of electronic communications to enable healthcare providers at different locations to share information for the purpose of diagnosis, treatment, education, and care management. The services may include, but are not limited to, electronic transmission of medical records, photos, diagnostic images, live two-way audio and video, output data from medical devices, and sound and video files.
2. Expected Benefits
- Improved access to medical care by enabling you to remain at your location while the physician consults from theirs.
- More efficient medical evaluation and management of your condition.
- Reduced time away from work or other obligations for medical visits.
- Ability to receive care when in-person visits are not practical or available.
3. Potential Risks
- Delays in evaluation or treatment could occur due to equipment failures or technical limitations.
- In rare cases, the transmitted information may be insufficient for appropriate medical decision-making, requiring a follow-up in-person visit.
- Security protocols could fail, causing a breach of privacy of personal medical information.
- There may be limitations in the ability to perform certain physical examinations remotely, which could affect diagnostic accuracy.
4. Privacy and Security
You understand that the laws that protect privacy and the confidentiality of medical information also apply to telehealth, and that no information obtained in the use of telehealth which identifies you will be disclosed to other parties without your consent, except as required or permitted by law. Our platform uses encryption and other security measures to protect your health information.
5. Your Rights
You have the right to withhold or withdraw your consent to the use of telehealth at any time without affecting your right to future care or treatment. You also have the right to receive in-person medical care, and the use of telehealth is at your discretion. You may request a copy of your medical records at any time.
6. Medical Emergency
If you experience a medical emergency, call 911 or go to the nearest emergency room immediately. Telehealth is not appropriate for emergency medical conditions. Our services are not a substitute for emergency care.
7. Prescription Policy
All prescribing decisions are made at the sole discretion of the treating physician based on their professional medical judgment and an evaluation of your health information. Not all conditions can be treated via telehealth, and not all patients will receive a prescription. Physicians reserve the right to decline treatment or recommend in-person care when appropriate.
8. State Licensing
The physicians providing telehealth services through Longevity Health Medical Group PA are licensed to practice medicine in Florida. You must be physically located in Florida at the time of your consultation. By proceeding, you represent that you are located in Florida.
9. Consent to Treat
By using our telehealth services, you acknowledge that you understand the information provided above and consent to receive medical evaluation, diagnosis, treatment, and/or prescription services via telehealth. You agree that you have read and understood this consent and that your use of the platform constitutes your electronic signature and agreement to these terms.
